GLP-1 Weight Loss

Semaglutide vs. Tirzepatide: Which Is Right for You?

Semaglutide and tirzepatide are the two leading prescription medications for medical weight loss, both available through IVUSE+ telehealth. They work in related but different ways, and the right choice depends on your goals, your health history, and a clinician's review. Here is how they compare.

At a glance

Compounded Semaglutide

Single-pathway GLP-1

A GLP-1 receptor agonist that mimics a natural gut hormone to reduce appetite, slow gastric emptying, and regulate blood sugar.

MechanismGLP-1 receptor agonist
Clinical resultAbout 15% average body-weight reduction over 68 weeks in the STEP 1 trial (semaglutide 2.4 mg)
IVUSE+ price$285/month flat-rate, no subscription
Compounded Tirzepatide

Dual GIP + GLP-1

A dual GIP and GLP-1 receptor agonist that targets two hormone pathways instead of one, which can mean stronger appetite suppression.

MechanismDual GIP / GLP-1 receptor agonist
Clinical resultAbout 21% average body-weight reduction over 72 weeks in the SURMOUNT-1 trial (tirzepatide 15 mg)
IVUSE+ price$360/month flat-rate, no subscription

How they work

Semaglutide activates the GLP-1 receptor. GLP-1 is a hormone your gut releases after eating that signals fullness to the brain, slows how fast the stomach empties, and helps regulate insulin and blood sugar. By mimicking it, semaglutide reduces hunger and "food noise" so it is easier to eat less.

Tirzepatide activates two receptors: GLP-1 and GIP. GIP is a second gut hormone involved in how the body handles fat and insulin. Targeting both pathways at once is why tirzepatide is often described as dual-action, and it is the leading theory for why it has produced larger average weight loss in clinical trials.

What the trials show

In its pivotal STEP 1 trial, semaglutide 2.4 mg produced roughly 15% average body-weight reduction over 68 weeks (Wilding JPH, et al. N Engl J Med. 2021;384:989-1002).

In its pivotal SURMOUNT-1 trial, tirzepatide 15 mg produced roughly 21% average body-weight reduction over 72 weeks (Jastreboff AM, et al. N Engl J Med. 2022;387:205-216).

In its SURMOUNT-5 trial, the first head-to-head comparison of the two, tirzepatide produced greater average weight loss than semaglutide, roughly 20% versus 14% over 72 weeks (Aronne LJ, et al. N Engl J Med. 2025;392:2225-2237). This confirmed directly what the separate trials had suggested. Both are gradual, sustained treatments rather than quick fixes, and individual results vary widely based on dose, adherence, and personal biology.

How to choose

Consider semaglutide if

You want a well-established, widely-studied GLP-1 option at a lower monthly cost, or you prefer to start with a single-pathway medication.

Consider tirzepatide if

Your primary goal is the largest average weight reduction and you are comfortable with the higher monthly cost of the dual-action medication.

Either way

A licensed clinician reviews your health history, medications, and goals to determine which is appropriate for you, and dosing is titrated up gradually.

Many patients also start on one medication and adjust over time based on how they respond and tolerate it. That decision is always made with your clinician, not on your own.

Common questions

Is tirzepatide more effective than semaglutide?

Yes, on average. In their pivotal trials, tirzepatide reduced body weight by about 21% over 72 weeks versus about 15% over 68 weeks for semaglutide, and a head-to-head trial (SURMOUNT-5) confirmed the difference directly, roughly 20% versus 14% over 72 weeks. Individual results vary, and your clinician can help set realistic expectations for your situation.

Which one costs more?

At IVUSE+, compounded semaglutide is $285/month and compounded tirzepatide is $360/month, both flat-rate with no subscription and with medication, telehealth consultation, and shipping included.

Can I switch between them?

Some patients switch based on how they respond, tolerate side effects, or progress toward their goals. Any change is made with your prescribing clinician, who adjusts your protocol and dosing accordingly.

Start with a clinician, not a guess

The best way to choose is a short telehealth review. Complete a confidential health assessment and a licensed clinician on the IVUSE+ Clinical Team will determine whether semaglutide or tirzepatide is appropriate for you, available to eligible patients in all 50 states.
